We accept new patient registrations at the following times:

8.00am to 9.00am

4.00pm to 5.00pm

You can collect a registration pack in person at the surgery or download and complete the forms below:
Bring your completed forms, along with:

Photo ID
-
Proof of address

Patients must attend the practice in person to register.

We only accept patients who live within our practice area boundary. (See our Practice Area page)

We treat all patients fairly and without discrimination on grounds of race, sex, religion, disability, or illness.

You will be registered with one of the practice GPs. If you wish to register with a specific doctor, you may request this — if not possible, an explanation will be given.
💻 Summary Care Record
All new patients will be asked whether they wish to have a Summary Care Record, which includes:

Medications
-
Allergies
-
Adverse reactions
This record helps provide safer care in emergencies, when the practice is closed, or when you're away from home in another part of England.

If you wish to opt out, please ask reception for a form.

Children will automatically have a record created — to opt them out, a parent/guardian must complete and return a form.
